How to Read Your Insurance Card
Your insurance card includes important information you’ll need when scheduling or checking in for an appointment. Review the sample below to see what to look for:

Key information on your insurance card:
- Member/Subscriber Name: The name of the person covered under the plan.
- Member/Subscriber ID #: Your unique identification number used to verify coverage.
- Group ID #: Identifies your specific insurance plan and helps determine your benefits.
- Copays (Primary Care, Specialist, Emergency Room): The amount you may need to pay out of pocket for different types of visits.
- Rx Group Number: Identifies your prescription drug coverage.
- Rx Copay: Your out-of-pocket cost for medications after insurance.
