Conexiones ITS provides medically monitored withdrawal management and clinical stabilization services.

At Conexiones, we understand that every individual’s journey through recovery and mental wellness is unique. Our Individualized Treatment Stabilization (ITS) service model is an integrated Acute Treatment Services (ATS) and Clinical Stabilization Services (CSS) program that prioritizes continuity of care and supports individuals from medically monitored withdrawal management (Detox) through clinical stabilization. Conexiones offers culturally rooted, individualized recovery care aimed at the unique needs of individuals living with co-occurring substance use and mental health disorders, and other chronic care needs. 

Our care plans weave together substance use, mental health, medical, and psychiatric treatment to address the holistic needs of our patients. Through an evidence and strength-based approach, we help individuals who have not found success with other treatment models stabilize and build motivation and engagement for ongoing recovery in the community.  

Length of stay is based on an individual’s needs and typically ranges from 5-7 days for withdrawal management and 14-21 days for clinical stabilization. For patients receiving both levels of care, the total stay generally ranges from 19-28 days. 

All services are available in Spanish and English. Other language needs are met through interpreter services.

Conexiones serves individuals who:
  • Adults (18+)
  • Massachusetts residents
  • Struggle with alcohol or other substance misuse   
  • Meet medical criteria for 24-hour inpatient ATS or CSS care (ASAM level 3.7 or 3.5)
  • Able to participate and engage in the treatment process 
Services:

All Acute Treatment Services and Clinical Stabilization Services in an integrated unit, including:  

  • Withdrawal management (Detox) as appropriate 
  • Integrated addiction, mental health, and medical/psychiatric evaluation and assessment 
  • 24/7 On-site nursing    
  • Medication reconciliation/monitoring    
  • Medications for Opioid Use Disorder (MOUD) 
  • Psychopharmacology services 
  • Evidenced-based individual and group counseling and skill-building 
  • Health and wellness education    
  • Peer-led self-help groups    
  • Individualized case management    
  • Aftercare planning     
  • Transportation support 
  • Access to peer recovery coaching and other support services
