Clinical Stabilization Services (Conexiones CSS)

We provide evidence-based substance use disorder treatment and integrated medical and support services.

Conexiones helps patients prepare for long-term recovery services.

Services Include:
- Medical and psychiatric evaluation and assessment
- Medication management and access to Medication Assisted Treatment (MAT)
- On-site nurse practitioner seven days a week
Acute Treatment Services (Conexiones ATS)

We provide bilingual (Spanish and English) evidence-based, medically monitored inpatient withdrawal management and wrap-around recovery support services.

Conexiones ATS-Detox helps patients manage acute withdrawal symptoms to support their for long-term recovery.

A licensed CARF-accredited behavioral health clinic with telehealth support.

Connects participants with recovery resources to reduce criminal justice involvement.

- Section 8 voucher or waitlist
- One or more physical/mental disability
- Recent homeless status
- At least one adult (18+) in household

- 14 Studio Apartments
- 8 Family Apartments
