Offender Reentry Program Coordinator

This is a grant funded position through the Substance Abuse Mental Health Services Administration (SAMSHA). The Transcend: Empowerment, Recovery, and Reentry project is an adult Offender Reentry Program, designed to provide inmates a coordinated approach to transition planning from the corrections to client-centered recovery and reentry services in a community-based setting; which includes (1) transitional support from corrections to community-based residential treatment; (2) integrated residential substance abuse, trauma, and mental health treatment; and (3) relapse and recidivism prevention, and aftercare planning and referral to support reintegration into the community.
The Program Coordinator will provide both direct service and administrative duties. S/he will provide counseling and case management services to the target population, including coordination of wrap-around recovery services. The Program Coordinator will work closely with both the Men's and Women's residential program to ensure the Transcend program is delivering and documenting the provision of quality services in compliance with funder requirements, agency and accreditation standards.
